12-lead ECGs that are recorded in the United States typically display simultaneous recording of 4 sets of 3 leads (leads I,II,III; aVR,aVL,aVF; V1,2,3; and V4,5,6). In the total population, the Cabrera sign presented the highest specificity (97%) and diagnostic accuracy (62.2%), with a sensitivity of 23.6%. The Cabrera sign was the only significant predictor of a prior MI [OR = 9.9, (95%CI:2.8–34.5), p < 0.001], among all ECG markers. Pacing site did not significantly influence the sensitivity and specificity of the Cabrera sign for detection of prior MI. Signe publié par E. A "fragmented QRS" is a sign of infarction, either new or old, analogous to Q-waves. ECG changes associated with Stage I pericarditis, first described by David H. Spodick in 1974. The sign remained relatively unevaluated until a recent retrospective analysis in 2020.

Herweg B, Marcus MB, Barold SS. Diagnosis of myocardial infarction and ischemia in the setting of bundle branch block 3. Barold SS, Herweg B. Cabrera's sign refers to a notch that occurs within the first 40 msec in the ascending portion of the S wave in lead V3 or V4: Cabrera's sign (arrowed) This ECG sign was first described by Cabrera & Friedland in 1953. Although Cabrera's sign has a high specificity for the diagnosis of an old myocardial infarction in LBBB, it has a poor sensitivity. The Cabrera format of the 12-lead ECG As evident in Figure 1, the traditional presentation of the ECG leads is not particularly helpful.

Cabrera sign (kă-brā′ră) Electrocardiographic evidence of myocardial infarction in patients with left bundle branch block or ventricular pacing, in which there is a 0.04 second notch in the ascending limb of the S wave of leads V3, V4, or V5.
The hexaxial reference system, better known as the Cabrera system, is a convention to present the extremity leads of the 12 lead electrocardiogram, that provides an illustrative logical sequence that helps interpretation of the ECG, especially to determine the heart's electrical axis in the frontal plane. Cabrera sign is defined as the presence of a prominent, shelf-like or downward notching, of >0.04 second duration, at the ascending limb of the S wave. Cabrera and Friedland in 1953 found that late notching of the up-slope of the S wave in leads V3–V5, is highly sensitive and specific for the diagnosis of old MI in LBBB. Signe de Cabrera. Notching of the ascending limb of the S wave in leads C3 or C4 is called Cabrera's sign. It is used to diagnose acute myocardial infarction in the presence of left bundle branch block (LBBB). In the presence of other clinical findings Cabrera's sign suggests the presence of acute myocardial infarction. Spodick sign: Downsloping TP segment. It is used in the assessment of previous myocardial infarction (MI) in the presence of left bundle branch block (LBBB) or paced rhythm. Standard textbooks have traditionally taught that the pathological Q-wave is a permanent ECG manifestation and that it represents transmural infarction (STEMI). However, recent studies challenge these notions. Pathological Q-waves may resolve in up to 30% of patients with inferior infarction. Tag: Cabrera’s sign.
